Some that don't seem very painful can be extremely so if held for a long time.
Speaking from experience, being tied on a table with your hands together above your head (your head is close to the edge of the table) and then pulled down slightly so that your hands are a little below the edge of the table with just a little give in the rope is not uncomfortable to begin with, but keep someone in that position for more than 30 or 40 minutes and the pain becomes excruciating, in the shoulders, neck and back.

She will beg to be released, offering whatever you want. She will offer to be spanked, whipped, caned, anything, to get out of that position.

This position is probably just the kind you are looking for, for a narrow shouldered woman that is, since men's arms and shoulders are slightly different in width, etc., it may not be as painful to a man, or it may be more.
